Firearms of medicolegal interest are the portable ones, whose great spread has made them the main means of causing injury.

We report the case of a nineteen-year-old man who came to our attention after a ballistic trauma.

From the investigations carried out on the patient’s lesions, it was assumed that the bullet entered from the left parasymphysis region, making a small hole without the typical cleansing border (probably something was put between the weapon and the point of impact) and blackening; then the bullet seemed to have fractured the left chin region immediately below the lower alveolar process, and it finally stopped in the submandibular area, in the superhyoid region of the neck.
